COVID-19 Vaccine

Please call the office if you'd like to schedule a COVID Vaccine appointment. We also currently have stock available to administer doses at your child's well visit.
If you are coming for one of our vaccination clinics, please read the following information to help the day run smoothly.
Prior to your vaccine appointment you will need to fill the NYS demographic and screening form:

Click here for the form for ages 6 months to 11 years old and here for the form for ages 12+which we encourage you to fill out in advance and print and bring to your appointment. Note, if we have your insurance information on file you do not need to fill out that section unless it has changed since your last visit. This form needs to be filled out for the primary series (1st and 2nd doses) and boosters.

Remember to bring your vaccine card with you to your appointment for your second dose and booster.
How to prepare for your visit

For ages 3 and up vaccines are typically given in the upper arm. Please dress your child appropriately to help speed up the process and allow us to administer the vaccination as efficiently as possible.

Please arrive no earlier than 15 minutes before your time slot and be prepared to remain under observation for 15 minutes after the vaccine is administered.

There will be a 15-30 minute observation period following the vaccination (depending on your health history), which will take place outside - please dress warmly.
While it is our goal to vaccinate as many of our patients as possible, if you are unable to make the appointment times we have available, or if they are all booked up, we encourage you to get your child vaccinated at another location. Schools, pharmacies and community based clinics are also administering the vaccine. You can use the CDC Vaccine Finder here to find alternative places to get your child vaccinated.
