Our office receives a great deal of forms, all of which must be looked at by a doctor. Please get all school, daycare, and camp forms to our office as soon as possible! Forms will be completed within seven days after we receive them in the office for a $5 fee. Otherwise, if you need your form expedited, we can have it ready for you in 24 hours for a $20 fee. Forms not picked up within 1 month will be shredded.
